Document Properties Lost

I have created documents with form fields and radio boxes. I need to use the documents as stand alone docs, but also to incorporate them into a larger manual. When I replace pages in the manual with the stand alone versions, all the formatting from text fields and radio boxes are gone. What am I missing that causes that? Shouldn't the properties carry over? I am using Acrobat 8 Pro. Does the fact that the stand alone docs have usage rights enabled have anythng to do with the problem?

I just tested making a copy of a usage enabled form and then replaced pages with that copy - same problem, so I doubt the rights enabled was the cause of the problem.
